Filling in the form
The decision to shortlist you for interview will be based solely on the information you provide on your form so it is important that you complete the form as accurately and as fully as possible, whether you are currently working for Dorset County Hospital NHS Foundation Trust or not.
Please complete the form in black ink or type, as it will be necessary to photocopy your application form. If you also wish to include a CV, we recommend that this should be a maximum of three sheets of A4 size paper.
Job Description and Person Specification
The Job Description outlines the main purpose and duties of the job. The Person Specification identifies qualifications, experience and skills that are necessary for the job and the criteria against which you will be selected for an interview.
Car drivers
If driving is a requirement of the job please give details of any endorsements on your driving licence.
Work Permits
The Trust can only recruit someone who needs a work permit if the vacancy has been extensively advertised and it has not been possible to recruit a suitably qualified person who does not require a work permit under the conditions of the Immigration Act 1971. You should note that if you had a work permit with a different employer you will need a new one to work with the Trust.
If you are not a British Citizen or European Community National and need a work permit, please give details on a separate sheet and attach it to your application.
Education, training and qualifications
Make sure you give all the information needed, including dates of study, making clear the level of examination (e.g. GCSE, GCE O Level or A Level) and the grades obtained. Also include any special skills training, short courses or relevant work placements.
Criminal background
Because of the nature of the work for which you are applying, this post is exempt from the provisions of Section 4(2) of the Rehabilitation of Offenders Act 1974, by virtue of the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975. You are therefore not entitled to withhold information about convictions, which for other purposes under the Act are “spent”. Failure to declare any and all past court convictions, bindovers and cautions, or any judgements or investigations pending may result in dismissal or disciplinary action by the Trust.
In the event that you are successful in your application, a Disclosure may be required from the Criminal Records Bureau (CRB) for the post for which you have applied. A list of the posts for which Disclosure is necessary and the levels of Disclosure is enclosed.
A past conviction will not necessarily be a bar to obtaining a position.

Employment history
Write here the name and full address of your present and past employers and the dates you were employed by them. Please give a full career history including any temporary, part–time, voluntary or unpaid work. Please give brief reasons for any gaps in employment.
References
Please give details of two people not related to you who can provide a reference for you. Your principal referee should be your present employer or tutor. A second reference is required from somebody who can testify as to your suitability for the type of employment for which you have applied, for example a past employer or tutor, or someone who can provide a character reference for you. Both referees will be contacted before interviews take place unless you clearly state otherwise.
Additional information
This is the most important section. You should give your reasons for making this application and your interest in this area of work.
You must demonstrate how you meet each of the criteria identified in the Person Specification drawing on your past experience and highlighting the skills you have gained and developed as a result. These should then be put into context by providing specific examples. Include any activities outside of work that may be relevant, for example voluntary, community or charity work; leisure interests; membership of professional or other organisations, or caring for relations.
If you use additional sheets, please number them clearly and print your name at the top of each sheet and finally remember to sign the form.
